这首词的背景是韦庄有一个资质艳丽、文化修养也很高的宠姬被蜀主王建强行夺去，韦庄追念此人，写了此词和小重山。此说法不一定可靠，很可能是为某一个分手以后消息不通的旧时情人而作。

诗歌史上，真挚、沉痛的悼亡之作甚多，都具有永恒的魅力。生活中一些有某种纪念意义的小物品，就会像一颗小石子投入池溏一样，在感情的波澜里激起层层涟漪。这首词通过一纸昔日情人的旧书信，在词人的心中荡起了层层涟漪，从而勾起了无数的回忆。全词以书信为中介，上片写读信后勾起的无数回忆，由此产生了渴望与意中人再传消息、寄书信的痴情；下片写思极而睡，醒来不忍再读伊人旧情书的情绪，并用景色加以陪衬。全词融纪实、写景和抒情于一炉，疏中见密又富有生活气息，脉络分明，情意真挚，将深切的悼亡之情表达得低回婉转，余韵袅袅。
这首词写一个男子对女子的怀想。上片着重勾画主人公的心理活动，是直吐衷肠。由回忆而动情，由动情而遐想，再传达殷切思念的痴情。下片侧重于人物形态和景物描写。“把伊书迹”是此词词眼。
上片着重勾画了主人公的心理活动。首三句，上句设问，下句作答，句句相联，环环相扣。首句“空相忆”，平地奇峰突兀而起，是主人公心理活动的基础。这三个字既笼罩全篇，又包孕无遗。一个“空”字不仅表现了“相忆”数量的以简代繁，而且写出了这种“相忆”的深和苦，“空相忆”是情至深处的痴人语。“无计得传消息”是紧承首句而答，意思是无法传递消息。第三句“天上嫦娥人不识”，是写欲向那位“天上嫦娥”传达殷切思念的痴情。“嫦娥”形容伊人体貌之美，“天上”；则暗示了伊人已仙去。至此，才看出相忆成空是由于生死相分，幽明相隔。第四句“寄书何处觅”与第二句“无计得传消息”意思大致相同，显示了要通款曲情意的执着和真切。向亡人通消息、寄书信，看似无理，实是深情的反应。这最后一句将自己虽欲寄书而无处寄书以至于最终不合瞄书的款款情意，以反问的形式加以表达，更是含蓄曲折，哀婉动人。
下片侧重于人物形态和景物描写，从而传出了幽深的隐曲心绪。“新睡觉来无力，不忍把伊书迹”两句以形传神，将上下片衔接得异常紧密。“新睡”二字暗示出是困倦已极的不由自主的睡。“无力”紧应“新睡”，睡醒之后应觉得有力，此言无力，自是伤心人别有怀抱。沈际飞说：“‘把伊书迹’，四字颇秀。”（《草堂诗馀正集》）“伊”字，口吻非常亲切。“把伊书迹”可见伊人兼有才情。“不忍”二字更妙，正写出“伊书”的感人至深，宜于表现出伤心人看与不看的矛盾心理。“把伊书迹”不仅“颇秀”，而且是这首词通体皆活的词眼。这句词语意淡而情深，故沁人心脾。“满院落花春寂寂，断肠芳草碧”两句别出心意，宕开一笔，以景作结。“满院落花春寂寂”是花落人亡的孤寂境。
